I am sure this question goes out often but I think it is always fun to see what people carry with em. Here is mine:

Here it is.  This is the main group of stuff in my bag which gets me by at most places.  If I am going out and need to actually be prepared it means I'm headed to an abandoned location.

Wow... I feel under prepared...  :o

In my camera bag (kata DC439 - a lot bigger than I need really - but I prefer to have space around the camera - less likely to change all the setting as i get it in and out..)

14-42 kit lens
25mm f1.4 pana
45mm f1.8 oly
(all lenses with hoods attached)
spare battery
spare 8gb sd card (16gb card in the camera)
lenspen mini

For model shoots (ie 80% of the time when I take my camera out) I also generally carry a second bag with:
5-in-1 reflector
bath towel (for drying any wet things I'd like a model to sit on)
plastic sheet (for things that I can't adequately dry / too dirty to bother - wooden benches, steps, etc)
manfroto tripod (for hanging reflector on, not for camera)
extra space to hold clothes, in case I can convince the model she's over dressed  8)

My bag and its contents vary depending on what I'm doing. For most hike or ski outings I use a Clik Elite fanny pack with at least 7 of the 10 essentials including water and a GPS receiver in the side pockets for easy access, and MFT gear inside. For longer treks I use a larger Clik Elite backpack, and for more civilized occasions (events, etc.) a shoulder bag.

I use a Think Tank Retrospective 5, which I love.  It is wonderfully made, canvas and fits a good kit comfortably.  I carry at least either my GH2 or my GX1 (and sometimes both, with my GX1 taking up my double lens compartment if I don't need many lenses, or without a lens and the GX1 in the outer pocket).  It carries 4-5 lenses comfortably and the outer pocket can fit lens hoods, a charger and my flash (or, as mentioned above, my GX1).  Love it. 

Generally I will carry one body with my 7-14, 25/1.4, 45/1.8 and one other lens (typically the 40-150 or 8mm fisheye, but also sometimes my Nikon 105/2.5 or Minolta 135/2.8.)  I just picked up a used 17/2.8 again on the cheap so I'll toss that in one of the compartments to always have with me.  Additionally, if I want to carry a larger lens, like my FD 200mm f/2.8, there are loop straps that I can attach another lens pouch that fits my big lenses on the side, just like it's part of the bag. 

If I'm traveling very light, I have a small little belt pouch that will fit my GX1 with either the 17, 25 or 45mm.  If I bring the 25mm with that setup, it's the only lens I can bring, but if I bring the 17 and 45, I can fit either lens in the outer pocket with the camera and other lens in.  Most of the time, I'll go out with the 17 and 45 with the GX1 when I want great image quality but a very small footprint.

When I'm really concerned about weight and don't need the ultimate in image quality, I just use my iPhone 4S. :)

Ha. Well, I go to some sketchy places sometimes.  Sometimes they are dark, dusty, dirty, chemicals, squaters and the like. It is not so different that the apocalypse if you come upon a bum in a broken down old mental hospital. Better safe than sorry with everything from physical protection to environmental protection.   And, like I said, this is my standard urban exploration kit.

I also have a Lowepro Sling bag which I toss a few essentials in, camera and two or three lenses.

I go out with 3 lenses MAX, in a small National Geographic waste pack.  One is on the Camera. And that is when I am doing a location shoot.
But, if you go on a day outing, and, you are not sure what you will be photographing (in the general sense), having a good arsenal of lenses, is a safe bet

Kata digital rucksack, laptop, writing materials, GH2 with 1 additional lens, lens cleaning kit. If I'm on the job the GH2 usually goes into one of my porta-brace production bags. LX5 in pants or coat pocket. In Brasil I never carry anything that looks remotely like a camera bag.

I like to keep it simple and light.
Olympus EP-1 with Voigtlander Color-Skopar 25mm f.4 attached for zone focused street style.
Panasonic G3 with Nikkor-H 50mm f.2 for head shots.
Olympus 40-150 for when I'm shy but still want to photograph people.
Olympus 17mm 2.8 for the big picture or autofocusing.
Spare batteries and the Lumix 20mm 1.7 if I'm going to be in the dark.
All this fits in my canvas shoulder bag which I've stuffed with a cut down,padded camera bag.It's very thin and light and I can carry this rig anywhere, all day long with no problem.

or post it on the gallery, the preview is much bigger and you have a much bigger size limit of 5 mb. Alternately, if you have flicker you can post it from there and itll look much bigger. Theres a tutorial on how to do so around in the getting started space.

I get it, I dont go to abandoned places here cause its really really dangerous, a stun gun wouldnt be enough.

I love the lowepro passport sling!!!!!!!!!! I can fit a house in there!!!

Stun guns are a joke.  My college room-mates and I used to stun each other all the time because we were bored and crazy.  The prongs you get hit with hurt more than the shock.  We actually got used to the shock, but I guess it depends on how much power it has.  Plus, in the cold miserable cities, most thugs wear thick clothing and are usually high on drugs that kill the pain anyways.  Mace is much more effective, especially the kind made to repel bears and cougars.  A flash light with strobe somewhere around 90 lumens or more also helps because in dark alleys it will temporarily blind the people.  As you walk buy a dark alley just strobe down it first to see if there any thugs.  Unlike what the media says, the economy is really bad and crime is going up.  I have family members that have been robbed this past year in some of the nicer areas of the bay.  Oakland will not investigate burglaries because it is considered low priority and they are understaffed.  It kind of is the end of humanity in some areas.

This bag will hold 3 lenses and a few batts... I carry my camera around my neck, never in a bag.

G3/20mm on neck

For an all around 4 lens kit, But, I rarely carry that much...

I hike with some people who pack firearms--a surprising number of people carry guns here--but for personal defense I just use my trusty Tilley hat...

When I'm specifically out shooting:

Olympus PEN Street Case M
Olympus 45/1.8
Olympus E-P3
Lumix 20/1.7
4Gb SD card
8Gb SD card (in the camera)
Lumix lens cap
Spare battery (had to put it in my E-P1 to take this pic)

But most of the time I just throw the E-P3 + 20/1.7 in my regular bag, or hang it around my neck/on my shoulder.
